PHOENIX – The Arizona Health Care Cost Containment System (AHCCCS) has selected Solari, Inc. for the administration of the Housing and Health Opportunities (H2O) program.
The H2O program is an innovative effort to enhance and expand housing services and interventions for specific populations of AHCCCS members who are experiencing homelessness.
“AHCCCS knows that housing is health care for our members,” said AHCCCS Cabinet Executive Officer Carmen Heredia. “The H2O program is an additional initiative we are prioritizing to improve member health and wellbeing, work to reduce homelessness and increase housing stability.”
With its launch this fall, the H2O program will initially start with those who have a Serious Mental Illness designation and are experiencing homelessness. In addition, eligible individuals must also have an identified chronic health condition.
Solari Crisis & Human Services is an Arizona-based non-profit organization for health care services that operates crisis and human services and contact centers. Solari is partnering with Banner Health for this contract.
“We are honored to enter this new partnership with Banner Health and to help the especially vulnerable population of Arizona residents with Serious Mental Illness designations who are experiencing homelessness,” said Justin Chase, CEO of Solari. “It is exciting to be at the forefront of this model and we hope that it can serve as a template for other states.”
The contract is effective October 1, 2024, for a contract term of three years with options to extend up to an additional two years.
