Overview
Arizona is submitting a state plan amendment to replace an outdated Advance Directive brochure with a link to the current document, located on the Arizona AG's website. The Arizona AG maintains and updates the Advance Directive documents required by the state. More information regarding the Advance Directive Update SPA, can be found below.

AHCCCS consulted with tribes regarding the Advance Directives Update SPA on July 11, 2019.
State Plan Amendment and Public Comment Period
Public notice was posted on July 29, 2019 and comments are being accepted through September 13, 2019. Comments can be submitted through email or mail at the address below.
