outlines the minimum program requirements and standards for Approved Programs to ensure uniformity of the
training and testing of DCWs and DCW Trainers as well as the portability of testing records.

Information on how to request standardized tests To request test questions, please send an email to
DCW@azahcccs.gov with the following information.
Name of Training and Testing Program
AHCCCS Provider ID number (if applicable)
First name, last name and title of the requestor
E-mail address of the requestor
Telephone number of the requestor
Attestation statement:
I will not share the bank of test questions and answers with any students or staff associated
with the training and testing program except for qualified trainers. I understand that failure to maintain the security of the
bank of test questions and answers can result in the termination of [insert name of company] program approval.
The primary purpose of the online database is to serve as a tool to support the portability
or transferability of DCW or DCW Trainer testing records from one employer to another employer.
A secondary purpose of the database is to support DCW Agencies in monitoring compliance with
the AHCCCS DCW training and testing initiative.
To initiate use of the online database, visit the “Training” tab on the left-hand side of
webpage. AHCCCS encourages Approved Program administrators to complete the computer-based
training courses to support them in developing internal protocols to institute the online
database into day-to-day business practices.
Fax Verification Form
The testing records of a DCW or DCW Trainer are portable from one employer to another employer.
Prior to the onset of the DCW Online Testing Records Database, employers of DCW Trainers could
use the Fax Verification Form to verify whether or not a DCW Trainer passed the competency tests.
During the transition to the online database, AHCCCS will permit the use of the Fax Verification
Form in the event an employee’s testing history is not found in the testing records database.
The new employer of the DCW Trainer must fax the form to the former employer to verify the testing
history and maintain a copy in the personnel file.
Audit Tool and Instructions
AHCCCS and ALTCS Contractors share responsibilities for auditing Approved Programs.
The audits are conducted to ensure compliance with AHCCCS policy and standards for training and
testing DCWs and DCW Trainers. The audit instructions and tools are used by the auditor to perform
the audit. That said, the instructions and tools can be used as a checklist to support
Approved Programs to assess compliance and prepare for scheduled audits.
